Please note you can fill out client consultation form prior to your appointment time, if you book a facial treatment, wax or lashes. This helps the Esthetician get to know your skin type, sensitivities, allergies, etc.

For any appointment that the client is under the age of 18, we ask to have an parent or legal guardian to fill out permission form to have on file. This will need to be filled out annually through gloss genius. This form will be sent 48- 72 hours prior to your appointment and when booked.
